​We will submit claims for you as long as we are a participating provider with your insurance and you provide the following requested information including:

1. Government issued photo identification (driver's license, military ID, etc)

 

2. Current insurance card

Co-payments, co-insurances, deductibles as well as prior balances, are expected at the time of your appointment. Cash, check, and credit cards (Visa, Mastercard, Discover) are accepted.

 

 

 

 

 

 

 

Mid Atlantic Retina Specialists submits to most major insurance carriers, including all out of network providers. If a referral is required by your insurance carrier, it must be provided at the time of service. Referrals must be obtained from your primary care physician.
